• Have had a positive Covid-19 Test in the last 10 days? 

• Have you recently been exposed to COVID-19? 

  • Are you experiencing any of the below symptoms: 

Fever >100.0 

Chills 

Cough 

Shortness of breath or difficulty breathing 

Fatigue 

Muscle or body aches 

Headache 

New loss of taste or smell 

Sore throat 

Congestion or runny nose 

Nausea or vomiting 

Diarrhea 

 

• Do you have any of the above signs and symptoms or have tested positive in the last 10 days? If so, we kindly ask you to notify the facility if you have recently been in to visit and defer your visit, if non-urgent, until 10 days after your positive test result.  

 

• If you are experiencing symptoms, we encourage you to test for COVID-19 as well as other respiratory illnesses in non-urgent situations prior to visiting your loved ones.  

 

• Visitors who has been exposed to someone confirmed positive for COVID-19 should wear a mask for the duration of their visit. If you are not able to wear a mask, if not urgent, we please ask your visit is postponed.
